Other treatment options available depending on patient

Item Special Note

Dental Services to be performed at 398 Commonwealth Ave Boston,
MA 02118 by Michael Faynzilberg DMD after August 1, 2013.

Services will begin with a comprehensive oral exam, full mouth 
radiography (unless a recent FMX is provided by the patient(s) 
[within 5 years]) and patient(s) education about their teeth, gums 
and oral health. Followed by Scaling and Prophylaxis (cleaning).


A comprehensive treatment plan will be discussed and agreed upon 
by the dentist and patient(s).

All treatment plans will be formulated in consideration of the 
patient(s) previous and current medical history. Previous and 
current: oral health, caries prevalence and periodontal health. If a 
patient(s) seeks a desired treatment that the dentist does not agree 
with, the dentist will make every effort to offer alternative treatment 
to satisfy the patient(s) desires. However, the final say of treatment 
to be performed is the responsibility and under the discretion of the 
dentist alone.
