Terms and Conditions:
* Certificate is available to first-time Club members only.
* Must be at least 18 years of age and photo ID is required.
* One certificate per person and no other offers, certificates, discounts, or packages may be combined with this Membership.
* Membership to be used exclusively at the specified Club beginning on or after date of purchase.
* The Membership may be transferred to another person prior to activation. Once activated it cannot be modified or transferred to another person.
* At the end of the Membership terms, Member may elect to continue the membership by paying the then current monthly dues rate for the membership type and signing the Club’s Membership Agreement.
* Membership must be activated at the specified Club by 12.31.2010.
* No cash value.
* Not refundable.
