Booking Instructions: Shop our web site, www.RoyalCaribbean.com, or our brochures to decide your cruise vacation. Please complete the information below and mail original form to Cruise Redemption Department. Please retain a copy of the completed form for your records and mail the original with delivery confirmation. Submit ship and sailing preferences based on Cruise Certificate Details included on this form. Once 
the information is verified and a booking is made, a representative will contact you to confirm the reservation. Cruise does not include air or ground transportation to the port of embarkation or items of a personal nature such as shore excursions, beverages, gratuities, photographs, medical services, etc. All necessary travel requirements, including documentation, are the sole responsibility of the cruise recipient. Taxes and fees, 
gratuities are additional, the responsibility of the cruise recipient, and must be paid before sailing. Eligible stateroom categories for this offer are G, H, I, L, M, N, Q. Guest can upgrade to a higher category for additional fees up to current prevailing rate, based on availability. Itineraries are subject to change without prior notice. All rules that apply to guests purchasing a cruise apply to the cruise certificate holder, including the terms and conditions listed in the brochures and Cruise Ticket Contract. desk for details. Cruise itinerary or product cannot be changed. Please contact Royal Caribbean International® at least 60 days prior to sailing in order to enter your reservation request. Request will be accommodated based on availability within 30 days of the sailing. Sailing must commence prior to the expiration date above. This certificate is valid for new bookings only and cannot be used for pre-existing bookings. This prize is nontransferable, and may not be given, bartered, or sold. No guest under 21 will be booked in a stateroom unless accompanied by an adult 25 years old or older, except for under 21 year-old married couples. Additional restrictions and conditions (including blackout periods) may apply. Cruise must be booked 7 days prior to sailing. Royal Caribbean International is not responsible for and will not replace lost, stolen, or destroyed certificates.
