Must be in overall good health and complete and sign all required paperwork. Consultation with doctor is required prior to treatment which may be done on the same day as treatment. Botox should not be administered to a pregnant or nursing woman, to anyone with a significant neurological disease including but not limited to Myasthenia Gravis, Lambert-Eaton syndrome, Amyotrophic Lateral Sclerosis (AML, Lou Gehrig's disease), or Parkinson's disease, or to anyone with a known allergy to the toxin ingredients or to human albumin.
Pick-Up, Delivery & Shipping Options: Winning bidders who are current Laurence families may only pick up items at the school. Winning bidders who are not current Laurence families will receive their items by mail and are responsible for shipping fees. We will ship U.S. Postal Service Priority Mail, insured for your protection, while still keeping costs at a minimum, which will be approximately $7.00 for a large envelope (gift certificates, etc.) and more for boxed items. Shipping fees will added to the purchase price of the items. There are no returns, refunds or exchanges on any items. All sales are final.
